Understanding the medical questionnaire
The Diver Medical questionnaire identifies when a medical evaluation is required. Complete it carefully and bring every page.
The clinician assesses your health in relation to diving. Suitability is a medical decision; further records or assessment may be needed before a conclusion.
What to bring and mention
Give the date of your planned course or trip when booking.
- The complete form from your operator or training organisation.
- Current medicines and relevant medical reports.
- Previous diving medical documents, if available.
- Recent illness, surgery or changes in health.
- Previous diving injuries or difficulties.
Allow time before the activity
Book early to allow time for assessment and any additional records. Experienced divers may need a review after a change in health or time away from diving.
For professional or occupational diving, ask whether we can provide the specific assessment required.
